SSM Health is seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our healthcare team in St. Charles County, Missouri. The FNP will provide high-quality primary care services to patients of all ages, emphasizing health promotion, disease prevention, and patient education. Key Responsibilities:

  • Conduct comprehensive health assessments, including patient histories, physical exams, and diagnostic tests.

  • Develop and implement individualized care plans based on patient assessments.

  • Diagnose and manage acute and chronic illnesses.

  • Prescribe medications and other treatments in accordance with state regulations and practice guidelines.

  • Provide preventive care, including routine check-ups, immunizations, and health education.

  • Collaborate with physicians, specialists, and other healthcare professionals to ensure continuity of care.

  • Educate patients and families on managing medical conditions and promoting healthy lifestyles.

  • Maintain accurate and up-to-date patient records in the electronic health record (EHR) system.

  • Participate in quality improvement initiatives and professional development activities.

Qualifications:

  • Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.

  • Current licensure as a Nurse Practitioner in the state of Missouri.

  • Certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).

  • Minimum of 2 years of experience in a primary care setting preferred.

  • Excellent clinical, diagnostic, and therapeutic skills.

  • Strong communication and interpersonal skills.

  • Ability to work collaboratively in a team-oriented environment.

  • Proficiency with electronic health records (EHR) systems.

Job Summary:

In collaboration with physician colleagues, demonstrates and provides professional nursing care to patients requiring an advanced level of knowledge and management of their nursing care in both inpatient and ambulatory settings.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

  • Assesses patient of any age presenting for health care and obtains a personal medical history, appropriate physical examination with recording of the resulting information.

  • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.

  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.

  • Addresses and responds to patient and staff requests for information in a timely manner, prioritizing patient requests and urgent needs.

  • Plans for patient care by making comprehensive clinical decisions based on advanced diagnostic skills and professional judgment, reviewing results of diagnostic tests and procedures, developing and implementing a treatment plan for the patient and/or family, and providing a wide range of interventions required to effect change.

  • Problem solving skills demonstrated by making competent advanced clinical decisions, utilizing appropriate resources, consulting with physician, as appropriate, and involving management where necessary.

  • Maintains effective methods of formal and informal communication with staff so that dissemination of information/administrative decisions are conveyed in a positive and supportive manner; motivates and supports staff to achieve department/contractual goals and operational benchmarks.

  • Monitors the effectiveness of interventions.

  • Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients.

  • Collaborates with multidisciplinary team members by making appropriate referrals. Facilitates referral of patients to other appropriate community healthcare facilities, agencies and resources.

  • Facilitates staff, patient and family decision making by providing educational tools.

  • Recognizes and utilizes opportunities to act as a change agent by introducing nursing practice innovations.

  • Evaluates nursing practices associated with specialty area, defines needs/problems with patient care and makes constructive suggestions that creates a positive patient outcome.

  • Promotes positive health behaviors and self-care skills through education and/or counseling.

  • Works in a constant state of alertness and safe manner.

  • Performs other duties as assigned.

EDUCATION

  • Completion of a formal post-basic education program from or formally affiliated with an accredited college, university or hospital (Before July 1, 1998) or a graduate degree from an accredited college or university (After July 1, 1998) with a concentration in the appropriate advanced practice nursing clinical specialty area.

  • Board certification by a national recognized certifying board in the appropriate population focus

  • Current or compact RN license in appropriate state

  • Appropriate APRN license or document of recognition in appropriate state

EXPERIENCE

  • One year nursing experience

PHYSICAL REQUIREMENTS

  • Constant use of speech to share information through oral communication.

  • Constant standing and walking.

  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.

  • Frequent sitting, reaching and keyboard use/data entry.

  • Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.

  • Frequent use of smell to detect/recognize odors.

  • Frequent use of hearing to receive oral communication, distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.

  • Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.

  • Occasional lifting/moving of patients.

  • Occasional bending, stooping, kneeling, squatting, twisting, gripping and repetitive foot/leg and hand/arm movements.

  • Occasional driving.

  • Rare crawling and running.
